Job Summary:


The Medical Social Worker is responsible for the initial and continuing assessment of home healthcare and hospice patients and family psychosocial needs, and implementation of interventions in accordance with the interdisciplinary team plan of care to assist the patient and family in dealing with the psychosocial aspects of illness. The Medical Social Worker reports to the Medical Social Work Manager.


Education and Experience:

  1. Masters level graduate of a school of social work accredited by the Council on Social Work Education preferred, Bachelors level graduate also considered dependent upon experience.
  2. Currently licensed as a Clinical Social Worker in the states of Rhode Island and Massachusetts (degree in social work or related field can be considered in lieu of licensing per discretion of hiring manager).
  3. Minimum one year experience in medical social work preferably in a home care setting.


  1. Knowledge of and ability to apply social work principles and practices while exercising independent professional judgment within the realm of medical social worker.
  2. Ability to contribute to the understanding and assessment of psychosocial factors in patient/family care.
  3. Knowledge of community resources and ability to access them as needed by the patient/family.
  4. Ability to counsel the patient/family and provide emotional support related to illness and recovery.
  5. Ability to assess the coping skills of the patient/family as they effect the patient/family’s ability to participate in the interdisciplinary care plan.
  6. Ability to participate effectively with other members of the team in developing the interdisciplinary plan of care.
  7. Ability to support and assist coworkers in dealing with difficult patient/family situations.

Job Specifications

  1. Full Time 40 hours per week. Exempt. Benefit eligible.
  2. Works out of Portsmouth, RI office, Monday through Friday.
